Our practice is committed to providing you and your family with safe, gentle, and state-of-the-art dental care. Our thoughtful dentists and staff put forth an extra effort to make sure that you are comfortable and happy every time you visit our practice.

 

YOUR FIRST APPOINTMENT

Our staff is committed to excellence in care, and committed to making every appointment in our office enjoyable and friendly. We ask that you arrive on time and we will be on time as well. If possible, send your forms (see headers on the left of this page) before your appointment. Our staff would like to have your forms one-two days prior to allow enough time to input your insurance information, and evaluate your medical information PRIOR to your appointment. This helps us stay on time for your office visit.

WHAT TO BRING

For your first dental appointment, remember to bring the following documents:

  • Completed Registration Forms
  • Insurance information
  • X-rays if applicable
  • A list of medications you are presently taking

Please alert MBDA dentists and staff if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.). The additional information will help Myrtle Beach Dental Associates dentists and staff provide the best care at the highest level of comfort.
